Your responses help local nonprofits and government agencies understand what help is needed, improve recovery efforts, and advocate for more resources. If you give permission, your information can also be shared with trusted organizations that are actively responding to the disaster so they can better connect you to services.

Important: The Connective Survey is not an application for assistance, but you will have the option to sign up to receive text messages through Connective Texts with updates about available resources, assistance, and safety information.
